Awana Children/Youth Midweek Activity

Welcome to the pre-registration form for Awana Children's midweek clubs! We’re thrilled to have your child join us for a season of fun, faith, and friendship. Please complete this form to pre-register your child/children for our weekly Awana Youth Clubs. There is no charge for attendance.

Awana Clubs begin August 20, 2025. Time: 6:00 pm - 7:30 pm.

You will need to provide the following information for each child being registered:

 • Full Name
 • Date of Birth
 • Grade
 • Emergency contact information

By pre-registering, you help us ensure a safe and organized environment for all participants. On the first night of club, we ask that you complete the paper registration form we will provide when you drop off your child/children.
